HEALTH INSURANCE
Paid By: St. Vincent's and Associate
Benefits: Comprehensive hospitalization and major medical coverage through Choice Plan administered by BC/BS; or St. Vincent's Managed Plan administered by BC/BS, paying greater benefits for preferred provider services.
Eligibility: Associates budgeted to work 32+ hrs/pay period.

ACCIDENTAL DEATH AND DISMEMBERMENT INSURANCE Paid By: Associate
Benefits: Accidental Death and Dismemberment coverage may be purchased on a pretax basis. Family members may be covered (if eligible) at varying percentages of the associate's coverage.
Eligibility: Associates budgeted to work 32+ hrs/pay period.

GROUP LIFE INSURANCE
Paid By: Associate
Benefits: Group rates through payroll deduction for Group Universal or Term Supplemental Life, individual and/or dependent coverage available.
Eligibility: Associates budgeted to work 32+ hrs/pay period.

BUSINESS TRAVEL ACCIDENT INSURANCE
Paid By: St. Vincent's
Benefits: Accidental death and dismemberment coverage up to $300,000 while on business travel.
Eligibility: All associates.

AUTOMOBILE AND HOMEOWNERS INSURANCE
Paid By: Associate
Benefits: Group rates for auto and homeowners insurance from Liberty Mutual.
Eligibility: All associates. Associates budgeted to work 32 hours or more per pay period may pay through payroll deduction.

FREE PARKING
Paid By: St. Vincent's
Benefits: In assigned areas.
Eligibility: All associates.

HOLIDAYS Traditional or PTO Option
Paid By: St. Vincent's
Benefits: New Year's Day, July 4th, Labor Day, Thanksgiving, Christmas, plus three Personal Days/year (based on date of hire).
Eligibility: Full-time associates. (Only 1 Personal Day may be taken during first 6 months.)

DIRECT DEPOSIT
Paid By: St. Vincent's
Benefits: All payroll checks are automatically deposited directly to the bank account specified by the associate.
Eligibility
: All associates are REQUIRED to provide bank account numbers upon employment for participation in Direct Deposit.

CREDIT UNION
Paid By: Associate
Benefits: Savings, loans, credit cards and other services available through membership in the University Credit Union.
Eligibility
: All associates, upon employment. Make application to the nearest University Credit Union office.

U.S. SAVINGS BONDS
Paid By: Associate
Benefits: Payroll deduction for regular purchase of US Savings Bonds.
Eligibility: All associates, upon employment.

HOSPITAL DISCOUNTS
Paid By: St. Vincent's
Benefits: Discounts on various services provided by St. Vincent's, including PHARMACY DISCOUNT, CAFETERIA DISCOUNT, AND HOSPITAL SERVICES DISCOUNT for uncovered hospital charges (as specified by policy).
Eligibility: All associates.

PERSONNEL HEALTH SERVICES
Paid By: St. Vincent's
Benefits: Health services are available for associates, including pre-employment physicals, health screenings, and free mammograms to female associates (based on age).
Eligibility: All associates.

EMPLOYEE ASSISTANCE PROGRAM (EAP)
Paid By: St. Vincent's
Benefits: Professional assessment and referral service for associates and their immediate family members experiencing personal or emotional difficulties that may be affecting job performance.
Eligibility: All associates and immediate family members upon employment. No charge for initial assessment and referral.

DAY CARE
Paid By: Associate
Benefits: High quality, on-site facility for children six weeks to five years of age. Cost subsidized by St. Vincent's.
Eligibility: All associates based on availability and full-time attendance of child.

FLEX CHOICE
Paid By: Associate and/or St. Vincent's
Benefits: Substantial tax savings on insurance premiums, unreimbursed medical expenses, and dependent care. Plan includes a Cash Back Option for full-time associates and a Vacation Sell Option for eligible associates.
Eligibility: Associates who are in a 32+ hour budgeted position/pay period.

SICK CHILD CARE
Paid By: Associate and St. Vincent's
Benefits: Drop off children for care at Huggs & Kisses between 6:30 am and 6:30 pm - convenient downtown location.
Eligibility: All associates. Hospital pays 2/3 of cost associate pays per child.

DENTAL INSURANCE
Paid By: St. Vincent's and Associate
Benefits: Coverage for preventive and restorative care. Two coverage options. Both plans administered by Blue Cross/Blue Shield.
Eligibility: Associates budgeted to work 32+ hrs/pay period effective 1st of month after 6 mos of employment.

LONG-TERM DISABILITY (CORE)
Paid By: St. Vincent's pays 60%. Associate may purchase additional 10%.
Benefits: Continuing partial income protection from the 91st day of a disability, up to a maximum benefit period.
Eligibility: Full-time associates, first day of month after 6 months of employment (if actively employed).

LIFE INSURANCE
Paid By: St. Vincent's and Associate
Benefits: Several different coverage levels are available. Coverage available for associate, spouse and children.
Eligibility: Associates budgeted to work 32+ hrs/pay period. Effective upon acceptance from insurance company. Cost based on age. Associate and spouse premiums paid through payroll deduction.

LONG TERM CARE
Paid By: St. Vincent's
Benefits: Wide range of personal care, health care, & social services for people who suffer a chronic disability or long-lasting disease. Coverage available for associate, spouse, parents, and parents-in-law.
Eligibility and Cost: Associates budgeted to work 32+ hrs/pay period.

EDUCATIONAL ASSISTANCE
Paid By: St. Vincent's
Benefits: Up to $4,000/year for approved educational programs for full-time associates, $2,000 for part-time associates.
Eligibility: Eligible after 6 months of employment. Full-time and part-time associates budgeted to work 32+ hrs/pay period are eligible. Benefits may be taxable.

SPECIAL LEAVE
Paid By: St. Vincent's
Benefits: Fully paid jury duty, bereavement leave and leave of Absence based on policy.
Eligibility: All associates (according to policy).

RETIREMENT PLAN
Paid By: St. Vincent's Associate St. Vincent's
Benefits: BASIC PLAN (CORE) Fully paid retirement income, based on salary and years of service. 5-year vesting. (Participation in this Defined Benefit Plan may affect IRA's.) 403(b) SAVINGS PLAN: Pre-tax savings through payroll deduction for retirement. Associate selects investment options of Tax-Sheltered Annuity (TSA) and/or Mutual Funds. Always 100% vested. MATCHED SAVINGS PLAN: Hospital matches $0.35 for every dollar you put into the savings plan, up to 5% of your W-2 earnings. 5-year vesting.
Eligibility: BASIC PLAN: Associates who work at least 1000 hrs/year, after 1 year of employment, automatically eligible January 1 or July 1. (Includes flexi if working 1000 hrs/year.) 403(b) PLAN: All associates may contribute from 1% to 20% (within legal limits) of earnings. MATCHED SAVINGS PLAN: Contributions may begin anytime. All associates who work at least 1000 hrs/yr after 1 year of employment. Begins first Jan. 1 or July 1 after 1 year of employment.

PAID TIME OFF Accrual Option
Paid By: St. Vincent's
Benefits: 0-4 yrs = 15 days/year 5-9 yrs = 20 days/year 10+ yrs = 25 days/year
Eligibility: Full-time associates may enroll January 1 after 1 year of employment. Not eligible for Wellness Incentive.

SERVICE AWARDS
Paid By: St. Vincent's
Benefits: Awards presented after each 5 years of continuous employment.
Eligibility : All associates who work the required number of hours.

RETIREMENT BONUS
Paid By: St. Vincent's
Benefits: $125/year for each full year of employment.
Eligibility: All associate who retire in good standing with a minimum of 10 years of service and minimum age of 55.
