Online Pre-Registration
To help cut down on time and paperwork of registration, pre-admission forms
can be filled out electronically on the Internet from our web site. Online
Pre-Registration forms are found in My St. Vincent's.
From there, you will be able to safely and securely enter the information needed
to register you for your procedure. Please keep the reference number given at
the end of your online registration and bring it with you to your Pre-Admission
Testing. Pre-Admission Testing Appointments
Within 14 days prior
to your surgery, you should call the pre-admissions nurse to make an appointment
for pre-admission testing (laboratory, radiology and other necessary pre-operative
tests). We highly recommend pre-admission testing rather than waiting until the
day of surgery. If a health concern is recognized due to your test results, it
can be addressed prior to surgery, usually without the need to delay your surgery.

On the day of
your tests, check in at Patient Registration in the main hospital lobby. After
registering, you will be directed to PAT on the first floor, across from O’Reilly’s
Café. Parking
Patients and visitors should park in the Women's
and Children's Center parking deck. A map of the hospital campus and parking decks
can be found here. What to bring
You will
need to bring the following information: - Insurance identification or certified
insurance forms
- Social security number
- Date of birth
- Address
- Medications
taken regularly
- List of Known allergies
- Money for Parking
What
to expect
On the day of your tests, check in at Patient Registration in
the main hospital lobby. After registering, you will be directed to PAT on the
first floor, across from O’Reilly’s Café. Tests should take approximately
one hour.
Accommodations
St. Vincent's Centennial Lodge is designed to provide patient's families with comfortable, close and convenient overnight accommodations. Although reservations are not required, they are accepted and may be made by contacting the Centennial Lodge reception desk at (205) 558-3800.
Financial Arrangements
If you do not have insurance, please
contact the Business Office prior to your admission at (205) 838-3910 and ask
for a financial counselor. Call between 8:00 a.m. and 4:30 p.m. Based on your insurance
coverage, you may be asked to pay your deductible or co-pay at the time of registration.
Payment can be made using cash, check, or credit card.
